Not always. The rule is about 2 defenders being goal-side of the attacker receiving the ball. Usually the goalkeeper is one of them, but he doesn't have to be.

It was offside, as the official rule says that there should be at least 2 players (including the goalkeeper) behind the player to whom the pass is made when the pass happens.

Very much like a real report, it's final. You don't hand in your paper and edit it after you've handed it. If you fail to see my point with an incomplete report being unnecesary and annoying then you are not worth the effort for me to type this.
